Abstract:

Background: Acute pyelonephritis is a severe upper urinary tract infection predominantly affecting elderly males due to anatomical and systemic predispositions. The increasing prevalence of antibiotic-resistant uropathogens necessitates effective, safe, and individualised alternative therapies.

Case presentation: A 69-year-old male patient presented with symptoms of acute pyelonephritis, including flank pain, fever, burning micturition, hematuria, and foul-smelling urine. Based on individualised symptomatology and classical Homoeopathic repertorization, Terebinthina oleum 30C and Cantharis vesicatoria 30C were prescribed. Adjunctive nutritional advice included increased hydration with barley water and light, renal-supportive diet.

Results: The patient demonstrated marked symptomatic relief within 48 hours, with normalisation of urinary symptoms and laboratory parameters by the end of one week. No antibiotics were used.

Discussion: The remedies’ selection was supported by classical materia medica and emerging immunological research validating their roles in urinary tract infections. Nutritional support complemented the homeopathic treatment, enhancing renal recovery.

Conclusion: This case highlights the potential of individualised homeopathic therapy combined with supportive nutrition in managing acute pyelonephritis in elderly patients. Larger controlled studies are encouraged to substantiate these findings.
