A comprehensive review of the efficacy of the homoeopathic medicines in the management of Chronic Suppurative Otitis Media (CSOM)

Background: Chronic Suppurative Otitis Media (CSOM) is a persistent inflammatory condition of the middle ear, often associated with recurrent ear discharge, hearing loss, and tympanic membrane perforation. Despite advances in conventional treatment, challenges such as antibiotic resistance and surgical invasiveness necessitate exploration of alternative therapies. Homoeopathy, with its individualized and non-invasive approach, has shown promise in managing chronic ear conditions.
Objective: To critically review the efficacy of homoeopathic medicines in the management of CSOM and analyze outcomes reported across various clinical studies.
Methodology: A review was conducted by screening literature published up to July 2025. Studies included case reports, case series, observational studies, and controlled trials focusing on CSOM treated solely with homoeopathic medicines. Outcomes assessed included cessation of otorrhea, tympanic membrane healing, and hearing improvement.
Results: Seven relevant studies were reviewed. Remedies such as Tellurium metallicum, Mercurius solubilis, Calcarea sulphurica, and Calcarea fluorica were frequently used. Individualized prescribing based on miasmatic analysis, repertorization, and totality of symptoms was central to treatment success. Most studies reported complete cessation of discharge, partial or full membrane healing, and audiometric improvements. Repertory-guided prescriptions and constitutional remedies showed significant impact, especially in chronic or recurrent cases.
Conclusion: Homoeopathic medicines offer a safe, effective, and individualized alternative in the management of CSOM. The therapeutic outcomes are strongly influenced by personalized remedy selection, highlighting the importance of individualized treatment, miasmatic understanding and reportorial analysis.
