Lichen simplex chronicus is a chronic pruritic skin disorder characterized by localized lichenified lesion resulting from repeated scratching or rubbing. It often develops secondary to an underlying pruritic dermatosis or psychogenic trigger, that exacerbates skin thickening and pigmentation changes. Although it is not life threatening, its repeated relapses, ongoing discomfort, and cosmetic issues severely reduce quality of life. Early detection and focused treatment are crucial to break the cycle of chronicity and prevent complications. Case summary: A 60 years old, male patient came with the complaint of intense itching, thickened and hyperpigmented skin lesion over the anterior portion of both knees and shin bones, clinically diagnosed as Lichen simplex chronicus. After detailed case taking, repertorization and consulting homoeopathic materia medica, Hepar sulphuris was prescribed. Follow up over 6 months showed gradual improvement of itching with resolution of lichenification, associated with improvement in general well-being. Documentation of the case was done in the form of photographs of affected area in every follow up and at the end, Modified Naranjo Criteria was used to assess the causal relationship between the Homoeopathic medicine given to the patient and the positive outcome. This case demonstrates the successful treatment of Lichen simplex chronicus through individualized homoeopathic treatment. Unlike conventional approaches, it adds evidence to the literature supporting constitutional homoeopathy as a therapeutic intervention for chronic dermatological conditions.
